Managing health concerns and figuring out the health care system can feel overwhelming, but there’s good news! You don’t have to do it on your own.
HealthSelectSM medical plan participants, including those enrolled in Consumer Directed HealthSelectSM, have access to a dedicated team of care management clinicians who can address all types of health-related questions.
Care management clinicians are available by phone to provide extra support and guidance at no additional cost to you.
A local support system built for you
The Texas-based clinician team takes a holistic approach to address your health concerns and questions rather than focusing only on one condition at a time. They recognize that a one-size fits all approach may not fit all. They make it a point to take your history, lifestyle and behaviors into account as they listen to your concerns and offer guidance.
How our clinicians can help
Clinicians can help you by answering health questions about asthma, cancer, diabetes, heart disease, high blood pressure, musculoskeletal conditions and pregnancy. Dedicated mental health clinicians are available to help with conditions including substance use, anger management, anxiety, depression, domestic violence, grief, post-traumatic stress disorder and stress.
Whether you have questions about an upcoming surgery or a recent surgery, need follow up support, or if you were recently diagnosed with, or are managing a condition, a clinician is available to support you and your family.
Clinicians can have a confidential conversation with you about:
- a newly diagnosed health condition,
- managing a health condition,
- understanding a procedure or hospital stay,
- considering residential treatment,
- following your therapist or physician’s treatment plan and
- health and wellness resources.
