Care and support to help you manage heart health and diabetes

When Judy, a HealthSelect of Texas® participant, was diagnosed with congestive heart failure, she was scared and unsure of her next steps. With a history of Type 2 diabetes making her health more complex, she decided to call Blue Cross and Blue Shield of Texas (BCBSTX) and connect with a care management clinician who could help guide her, step by step.

Personalized care plan

During their first conversation, Judy’s clinician, Ian, reviewed her health history, medications, diet, activity level and financial needs. Together, they created a care plan with goals that would work for her.

Ian took the time to explain Judy’s new medications and side effects to watch for, and he made sure she understood her diet and activity recommendations. They also discussed why it’s important to monitor blood pressure, track weight, watch salt intake and check her blood sugar regularly.

After finalizing Judy’s care plan, Ian helped ensure she had a referral to a cardiologist on file and scheduled a follow-up appointment for her. Before ending their call, Ian also told Judy about two wellness programs, Hello Heart® and Omada® for Diabetes, available through her HealthSelectSM medical plan. Judy was excited to learn that the programs were completely covered by her medical plan at no additional cost to her. She applied right away.

Making progress with digital tools

During their follow-up call, Judy shared with Ian how much the programs were helping her. With Hello Heart, she tracked her blood pressure daily and receive recommendations when it was high. With Omada for Diabetes, she kept her diabetes in control by checking her blood sugar levels regularly and using the app for personalized coaching and education. Judy especially liked being able to share her results from the Hello Heart and Omada apps with her health care providers.

Gratitude for ongoing support

Judy thanked Ian for his support and the resources he shared, which had turned an overwhelming medical diagnosis into something she felt she could handle. She now felt empowered to better manage her health.
