OnMyWay Health Assessment

When first logging in through Blue Access for MembersSM, you will receive a reminder to complete your Health Assessment if you haven't yet done so. The OnMyWay Health Assessment, sometimes known as a health risk assessment, takes about 15 minutes and is made up of adaptable questions to learn more about your current health status, including how your health compares with others in your age group. It will identify what you are already doing well and where there are opportunities for improvement. After you take the assessment, you will get a personal wellness report with recommended goals and action steps. You also can compare your results with previous reports! Your answers will help tailor the Well onTarget portal with the programs that may help you reach your goals.

Check with your benefits coordinator to see if your employer allows you to receive wellness incentives for completion of the Health Assessment. Once you take the Health Assessment, you can print a certificate of completion. You can also earn 2,500 Blue Points for completing your Health Assessment (once every 6 months).

Well onTarget Portal

The Well onTarget portal is tailored to your interests and activity, or non-activity, within the portal. 

With My Journey, you have access to a set, or “journey” of recommended activities. Examples of activities that make up journeys are actions like taking the Health Assessment, having a biometric screening, watching a video on a suggested clinical topic or connecting a device to help track fitness. As you complete different activities, journey options appear in the form of explore cards.

Explore is a set of “cards” that show on the dashboard and that supply dynamic, personally relevant content and engagement opportunities. Cards are refreshed regularly based on your interests and completed activities.

Self-management programs

There are 26 digital, self-management programs available through Well onTarget to support your health journey. These programs will be recommended to you through Well onTarget based on your Health Assessment responses.

These programs provide a structured set of activities, support resources as well as clinical and behavioral information pertinent to your condition(s) and needs.

There are courses on a variety of subjects available through Well onTarget. Some courses, like Quitting Tobacco, Weight Management, and Managing Stress, are designed to help you improve your health. Others are meant to help you maintain a healthy lifestyle, like Preventive Health and Sleep Health.

The programs are either interactive or educational:
  • Interactive programs will involve creating daily habits to track, as well as a midpoint and final assessment.
  • Educational programs don’t include the daily habits or midpoint and final assessments. They include quizzes, podcasts and other resources embedded within a six lesson format.

Health and wellness tools and trackers

Well onTarget offers many wellness resources, plus syncs with many fitness devices. You can access:

  • articles, tools and condition management information,
  • an interactive symptom checker and
  • track a wide range of health factors from nutritional choices and stress levels to blood pressure and tobacco use.

Wellness coaching

Certified health coaches are available by phone or secure messaging through Well onTarget.

Personalized coaching can help you reach your goals. Coaches can:

  • help you quit tobacco or get support for maintaining your tobacco free status,
  • help you improve your physical fitness, nutrition, blood pressure or cholesterol,
  • put you in touch with a registered dietitian who can help you understand your relationship with food,
  • share ideas for making healthy eating choices,
  • design a health and wellness plan that's right for your fitness level, lifestyle and goals and
  • find creative, healthy ways of thinking and acting that could combat stress

Well onTarget's mobile app — AlwaysOn — allows you to:

  • Take your Health Assessment
  • Set personal health and wellness goals and track your progress
  • Connect with a wellness coach through secure messaging or by using the click-to-call feature
  • Track data synced from more than 80 fitness devices and apps
