VirtualCheckup® by Catapult Health

Make getting your annual checkup easier with a  VirtualCheckup® by  Catapult Health.

A VirtualCheckup allows all HealthSelectSM medical plan participants 18 and older to complete their annual preventive checkup from the comfort of their own home.

Find information about the VirtualCheckup by clicking on the questions below.

All HealthSelect of Texas® medical plan participants age 18 and older living in the United States, including those enrolled in Consumer Directed HealthSelectSM, who have not had an annual preventive checkup with another provider in the last 12 months are eligible to participate.

Important notice for women: Catapult Health recommends all pregnant women visit their regular OB/GYN for all prenatal care, including blood tests. Participation in the VirtualCheckup is not available for pregnant women. This VirtualCheckup is also not recommended for women who’ve had a double mastectomy with bilateral lymph node removal. If either of these apply to you, please continue regular care with your PCP.

Your health plan understands the benefits of preventive checkups and partnered with Blue Cross and Blue Shield of Texas (BCBSTX) and Catapult Health to provide you with the opportunity to participate in a virtual preventive checkup at no cost to you! You will learn accurate, actionable information about yourself and your risks related to diabetes, heart disease and stroke. A Catapult nurse practitioner will answer your health care questions and review your results through a private video consult. A VirtualCheckup can help identify health problems before they start. Preventive checkups may help you live a healthier life.

Your checkup includes:

  • lab-accurate results, blood pressure, height, weight, body mass index and abdominal circumference,
  • a detailed Personal Health Report available on a secure portal following your confidential video consultation and
  • a review of current medications, health conditions and your Personal Health Report with a certified nurse practitioner.

A VirtualCheckup does not replace an annual wellness exam with your PCP. Catapult Health checkups focus on preventive measures, whereas an annual wellness exam from your PCP may include more comprehensive tests and services that can be performed in a doctor’s office setting. Catapult Health can share your preventive checkup results with your PCP if you choose to provide them with your PCP’s name and fax number during your checkup. This will help ensure your PCP does not request duplicate tests.

If you have had a visit with your PCP within the last 12 months, you may have already had the lab work you need. If your PCP has already done the lab testing, this virtual preventive checkup may not be needed. If you are unsure, check with your PCP.

No. If you plan to schedule a Catapult on-site preventive checkup, you should not register for a VirtualCheckup Home Kit. You are encouraged to participate in only one Catapult preventive checkup per year.

To order your VirtualCheckup Home Kit, visit VirtualCheckup.com/HealthSelect and follow the prompts on the screen. You should get your Home Kit within a few days at the mailing address you provided. Your Home Kit will be filled with useful items to complete your preventive checkup.

You will be asked to enter your personal information, including your name, date of birth, mobile phone number, ZIP code, HealthSelect medical ID number and email address. Catapult Health will send confirmation and email reminder messages to your email and text message. Your name, date of birth and medical ID number are used to identify you and will allow access to your appointment if you need to change or cancel it.

It is important that you enter your name and medical ID number exactly as it appears on your HealthSelect medical ID card. If the information you enter does not match what appears on your ID card, your registration may not be successful, and you will not receive your Home Kit.

Your Home Kit will include:

  • user instructions,
  • measuring tape,
  • virtually painless blood collection device,
  • compact reusable hand and arm warmer,
  • other supplies needed,
  • blood pressure monitor if requested during registration and
  • a pre-paid return envelope.

You can have your private video consultation on your smart phone, tablet, laptop or desktop computer. This is the preferred way for you to complete your Catapult nurse practitioner consultation.

If you do not have one of these devices, you can have your nurse practitioner consultation on your phone. If you need a phone consultation, email support@virtualcheckup.com. If you have your nurse practitioner consultation on your phone, you will have to complete your health history questionnaire online prior to having your nurse practitioner consultation.

Yes. Your individual results are strictly confidential and will not be shared with your employer. Catapult, your employer and BCBSTX take the privacy of your checkup results seriously. Yes. As a HealthSelect participant, you have many resources to help you stay healthy. These include wellness, weight management and care management programs to help you manage your physical and mental health. Your Personal Health Report will include information on how to access these resources. If you have further questions on how to find your health and wellness resources, call a BCBSTX Personal Health Assistant or visit healthselectoftexas.com

The nurse practitioner cannot refer you for services, like other preventive care services, or give prescriptions for medications. If you would like to discuss a specific condition or illness or if you need to schedule additional preventive care services, you should visit your primary care provider (PCP). If you are enrolled in the HealthSelect of Texas medical plan, only your PCP can refer you to another provider for care. If you don’t have a PCP, a Blue Cross and Blue Shield (BCBSTX) Personal Health Assistant can help you find a PCP and even help schedule a new-patient visit.

You will receive a Personal Health Report following your nurse practitioner video consultation. Your Personal Health Report includes:

  • test results that are easy to understand,
  • a Personal Action Plan with steps to help you improve or maintain your health created by you and the nurse practitioner,
  • a completion certificate and
  • recommended health improvement programs available through your HealthSelect medical benefits.

If you need to change or cancel your appointment, click the scheduling link provided to you by email, click “Change Existing Appointment” and follow the on-screen steps.

If you need to reset your PIN or request a new access code, email support@CatapultHealth.com

Catapult Health sends BCBSTX the results from your checkup. BCBSTX offers additional programs, such as the care management program, which can help you manage your physical and mental health. The care management program is a team of dedicated clinicians using a personal approach for addressing your health-related questions. The clinicians work with you and your health care providers to make sure you get the information and care you need. The BCBSTX clinicians can help provide clinical guidance, find community resources, schedule medical appointments and much more. If you get a call from a BCBSTX clinician, know that you will be in good hands if you choose to take advantage of this benefit.
