Make getting your annual checkup easier with a VirtualCheckup® by Catapult Health.

A VirtualCheckup allows all HealthSelectSM medical plan participants 18 and older to complete their annual checkup from the comfort of their own home.

A VirtualCheckup can help you learn if there are any specific health conditions you should ask your primary care provider (PCP) about. Catapult Health can also share your results with your PCP if you choose.

Important notice for participants in the Houston area: If you have selected a PCP in the Kelsey-Seybold group, you may be responsible for out-of-pocket costs if you participate in the VirtualCheckup program. For this reason, participants with a Kelsey-Seybold PCP are encouraged to see their PCP for all preventive services.

Three easy steps

A VirtualCheckup is fast and easy and available at no cost to you. Complete your checkup in three easy steps:

  1. Request your Home Kit, and it will be mailed to you within a few days. You decide where and when to complete your checkup.
  2. Follow the simple directions for your VirtualCheckup and then return it in the prepaid envelope.
  3. Schedule your private video consultation with a Catapult nurse practitioner. You can use your smart phone, tablet or computer for the consultation.

Your nurse practitioner will discuss the results of your checkup, give you advice and practical solutions and help you create a Personal Action Plan to live your healthiest life.
